• Requisition Form for MRI/CT/Radiology/Nuclear Medicine
  Requisitions must be signed by a physician and faxed or mailed to us. For urgent MRI requests, please contact the MRI radiologist at 604.822.7720. After hours, contact UBC Hospital switchboard for the radiologist on call, 604.822.7121.
